If you require any vaccinations relating to foreign travel you will need to fill out our travel questionnaire and hand it in to the surgery or email them to the practice.

A link is provided below;

Travel Questionnaire

When it is received our practice nurse will call you to book an appointment to discuss your travel arrangements.

PLEASE REMEMBER TO BOOK YOUR APPOINTMENT AT LEAST 5-6 WEEKS IN ADVANCE

There is further information about countries and vaccinations required on the links below

It is important to make this appointment as early as possible; at least 6 weeks before you travel.

Some vaccines need to be ordered as they are not a stock vaccine in addition some vaccines need two appointments.

Please remember in some cases vaccines need to be in your system for at least 2 weeks before you travel to allow them to work.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
